A national survey released in August shows employers tying vendor fees to results and expanding centers of excellence as costs climb. For conservative spine treatments such as motorized decompression, the new question is not just whether care is gentler than surgery, but whether it produces measurable results.

Picture a benefits manager at a Central Florida employer reviewing next year’s renewal. Musculoskeletal claims sit near the top of the list again, alongside heart disease and diabetes. The vendor pitches all promise savings. The manager’s question is blunt: which of these can prove it?

That question now applies to back pain care, including non-surgical spinal decompression. The direct answer is that employers are increasingly paying for documented outcomes, so conservative spine care is being judged on what it can measure: pain, function and time back at work. For many patients a nonsurgical first step is reasonable, and the clinics that track results carefully are best placed to show its value.

The shift is visible in new data. On Aug. 25, 2026, Business Group on Health published the executive summary of its 2027 Employer Healthcare Strategy Survey, drawn from 127 employers representing 11 million covered lives. It reports a median 8.5% cost trend expected for 2026, rising to 9.2% the following year.

Why are employers rethinking how they pay for back care?

Employers are rethinking back care because costs keep rising and they want results they can verify. The Business Group on Health survey identifies musculoskeletal, cardiovascular and diabetes as highly prevalent top three conditions driving cost, with cancer now the leading cost driver for 70% of employers.

The response is accountability. Eighty-four percent of employers plan to offer at least one center of excellence in 2027, and some are considering requiring employees to use one to receive coverage. The survey also found 83% of employers expanding the scope of performance guarantees and 71% increasing the share of vendor fees tied to outcomes. In plain business terms, the market is moving from paying for activity to paying for results.

What is non-surgical spinal decompression, and how is it different from surgery?

Non-surgical spinal decompression is a form of motorized traction in which a computer-controlled table applies a measured pulling force to the spine while the patient lies clothed. Surgical decompression, by contrast, removes bone or disc tissue in an operating room to relieve pressure on a nerve.

The U.S. Food and Drug Administration classifies powered traction equipment as a Class II device that requires a 510(k) submission; clearance allows marketing but is not a finding of superiority. Surgery remains the appropriate route for certain problems, such as severe or worsening nerve compression, and the two are not simple substitutes.

Is there a return on investment for conservative spine care?

There is no published return on investment figure for motorized decompression specifically, so any ROI claim should be treated with caution. What the data do show is why payers care about the question.

A retrospective analysis in the International Journal of Spine Surgery, published in January 2026, drew on a national insurance claims database covering 347,053 Medicare patients from 2010 to 2021, along with Medicare fee schedules. The authors found overall spine surgery utilization rose 19%, with cervical procedures up 30.61% and lumbar procedures up 19.97%. From 2012 to 2024, Medicare reimbursement for lumbar procedures fell 28.56%, and decompression procedures fell 27.38%.

Rising volume at lower unit prices is the pattern that pushes purchasers to ask whether some procedures could be avoided or delayed through well-chosen conservative care. That logic supports investing in good triage and first-line care. It does not, by itself, prove that any particular device lowers total costs.

Do health plans cover motorized decompression?

Coverage is often limited, which means many patients pay for decompression directly. UnitedHealthcare’s medical policy on motorized spinal traction, effective May 1, 2026, classifies these devices as unproven and not medically necessary for neck and low back disorders, citing insufficient evidence of efficacy. The policy’s evidence summary points to common weaknesses in the research, including small samples, poor blinding and short follow-up.

For employers and patients, the takeaway is practical: better-designed studies and consistent outcome tracking are what would move a service like this into performance-based contracts.

What do the survey and the spine data not show?

The employer survey and the Medicare analysis describe market pressure, not what happens to an individual patient. The Business Group on Health figures come from 127 large employers and report plans and intentions, not results, and the summary does not specify which conditions centers of excellence cover.

The spine surgery study relied on claims and fee schedule data, which capture billing, not pain or function. It cannot say whether any surgery was unnecessary or whether a conservative alternative would have worked instead. Coverage policies, too, reflect a judgment about current evidence and can change as better trials appear.

How is conservative spine care measured in South Orlando?

Measurement is where conservative care can meet the new standard. Many of those roles involve long hours on the feet, lifting and repetitive motion.

In a value-minded model, care begins with an evaluation that screens for red flags requiring medical or emergency referral, including loss of bladder or bowel control, numbness in the saddle area, progressive weakness, unexplained weight loss, fever or recent major trauma. When conservative care is appropriate, baseline and follow-up measures such as pain ratings, disability questionnaires and return-to-work status give both patient and payer something concrete to judge.

The business case, stated plainly

The trend lines point in one direction: purchasers want proof. For conservative spine care, that is an opportunity as much as a challenge. Clinics that document outcomes honestly, refer out when care is not appropriate and report results the way employers now expect will be easier to trust, and patients will have clearer information about what their time and money are buying.
